Occupational Therapist- Home Health

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full‑time or per‑diem position performing home health visits for the Vermont Visits office in Rutland, VT. The office serves adult and geriatric patients throughout Franklin and Chittenden Counties.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
• Evaluate the home environment for hazards or barriers and identify equipment needs and environmental adaptations.
• Continually assess and revise the occupational therapy care plan and collaborate with nursing on the multidisciplinary care plan.
• Educate and instruct patients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.

Qualifications

• Current Vermont Occupational Therapist license.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Prior home‑care experience preferred but not required.

Pay

Base Pay: $45.00 – $51.00 per point, depending on qualifications.
