Occupational Therapist (Home Health)

Role: PRN Home Care Occupational Therapist providing compassionate, patient-centered therapy in clients’ homes. Key Responsibilities Conduct functional assessments to evaluate needs and abilities.Develop individualized treatment plans based on assessment findings.Deliver OT interventions to improve safety, mobility, and independence in daily activities.Collaborate with an interdisciplinary care team to coordinate services.Educate patients and caregivers on therapy goals, strategies, and home routines.Maintain accurate, thorough, and compliant clinical documentation.
