Occupational Therapist - Home Health

About the role

Assess and evaluate patients' functional status, daily activities, fine motor skills, home environment, and adaptive equipment needs in a home-health setting. Deliver equitable, quality care across multiple counties.

Responsibilities

• Provide services within the scope of practice, following evidence-based techniques and the plan of care.
• Evaluate patients' functional and system status, consult with physicians, and develop therapy plans.
• Monitor and report patient responses and condition changes to the healthcare team.
• Conduct assessments using diagnostic and prognostic functional ability tests.
• Operate effectively in diverse home environments, regardless of patient demographics.

Requirements

• Current Occupational Therapy licensure in the state.
• Valid driver’s license, vehicle insurance, or access to transportation.

Preferred Skills

• CPR certification or ability to obtain within 90 days.
• Ability to manage multiple tasks independently.
• Strong communication, organizational, and writing skills.
