Occupational Therapist - Home Health

Responsibilities

• Assess and evaluate patients' functional status, activities of daily living, fine motor skills, and home environment needs.
• Develop and implement therapy plans in accordance with state laws, evidence-based practices, and physicians' guidance.
• Monitor and document patient responses to treatment, reporting changes to the healthcare team.
• Conduct functional ability tests and provide recommendations for adaptive equipment and home modifications.

Qualifications

• Current Occupational Therapy license and CPR certification.
• Valid driver's license, vehicle insurance, or access to transportation.
• Ability to work effectively in diverse home settings and with all client demographics.
