About the role

Plans, organizes, and implements Occupational Therapy services for PACE participants as a member of the interdisciplinary team. Responsibilities include assessment, treatment planning, therapy delivery, and education of participants, caregivers, and family to support community-based living.

Responsibilities

• Conduct initial and periodic in-person assessments to evaluate participants’ functional abilities, home environment, and rehabilitation needs.
• Recommend and coordinate adaptive equipment, home modifications, and therapy services to promote safety and independence.
• Develop, implement, and revise individualized treatment plans based on participant progress and interdisciplinary-team collaboration.
• Deliver occupational-therapy interventions in the PACE center, clinic, participants’ homes, and contracted facilities.
• Educate participants and caregivers on equipment use, home-safety strategies, and activities-of-daily-living programs.
• Maintain timely, accurate documentation of assessments, treatment, and participant progress per regulatory and organizational standards.
• Actively participate in daily interdisciplinary-team meetings, contributing to comprehensive care planning and 24-hour care coordination.
• Supervise Certified Occupational Therapy Assistants (COTAs), including review of care plans, co-signing documentation, and providing ongoing mentorship.
• Train staff in safe occupational-therapy practices and equipment use to enhance participant care.
• Perform integrated interdisciplinary-team functions encompassing the Occupational Therapist role and the Home Care Coordinator role, ensuring cohesive planning, communication, and oversight of both rehabilitative and home-based support services.
• May perform the duties of other interdisciplinary-team members based on professional licensing, competencies, and experience as needed.
• Support quality improvement, utilization review, and compliance initiatives.
• Maintain current licensure, certifications, and participation in ongoing professional development.

Requirements

• Advanced degree in Occupational Therapy from an accredited school.
• Current state license to practice Occupational Therapy.
• Two years of experience working on an interdisciplinary team in a hospital, nursing home, or community-based setting (preferred).
• One year of experience working with a frail or elderly population (preferred; training provided if not present).
• Competency in position-specific skills must be met prior to assuming participant care.

• Reliable transportation, valid driver’s license, and minimum state-required liability auto insurance.
• Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
• Comprehensive criminal background check, including federal and state Medicare/Medicaid exclusion lists, criminal history, education and license verification, reference check, and drug screen.
