About the role

Greenfield Rehabilitation Agency is seeking compassionate Occupational Therapists (OTRs)—Full-Time, Part-Time, or PRN—to join their mission-driven team serving their exceptional skilled nursing community in Verona, WI.

Responsibilities

• Evaluate residents’ physical, cognitive, and emotional functioning
• Develop and implement individualized, functional treatment plans
• Promote independence in everyday activities—dressing, grooming, feeding, mobility, and meaningful routines
• Support safe transitions, equipment needs, and environmental adaptations
• Educate caregivers, staff, and families to enhance outcomes
• Collaborate closely with interdisciplinary team members
• Deliver skilled therapy services across skilled nursing and assisted living settings
• Complete accurate and timely documentation

Qualifications

• Current Wisconsin Occupational Therapy license (or eligibility)
• Passion for supporting older adults in maximizing independence and quality of life
• Strong clinical thinking, communication, and teamwork skills
• A positive, flexible, resident-centered mindset
