About the role

Greenfield Rehabilitation Agency is expanding into a new senior community in Janesville, Wisconsin. We are seeking a passionate Occupational Therapist (OT) to join our team in either a full-time or PRN role.

Responsibilities

• Perform comprehensive occupational therapy evaluations and develop individualized treatment plans
• Deliver high-quality, evidence-based occupational therapy services
• Improve residents' independence with activities of daily living (ADLs)
• Address strength, coordination, cognition, safety awareness, and functional mobility
• Recommend and train residents in adaptive equipment and home safety strategies
• Collaborate with an interdisciplinary team focused on exceptional outcomes
• Build strong relationships with residents and families
• Contribute to a positive, team-driven therapy culture
• Maintain accurate, compliant documentation

Requirements

• Licensed Occupational Therapist (OT) in Wisconsin
• Strong clinical reasoning and treatment planning skills
• Excellent communication and team collaboration abilities

Qualifications

• Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Occupational Therapy
• License as required by the State of Wisconsin to practice as an Occupational Therapist
