OC FSP HFW Facilitator

About the role

The Facilitator ensures that the Wraparound process is implemented with fidelity and integrity, consistently maintaining hope that each family can achieve a better future. This role is responsible for initiating engagement with parents and youth, forming the Child and Family Team (CFT), developing safety plans, and creating an individualized CFT plan based on family strengths, needs, and preferences.

Responsibilities

• Actively participate in the engagement process by providing reassurance that the family’s circumstances and support the team in identifying and celebrating achievements, growth, and strengths throughout the Wraparound process.
• Schedule, convene, and facilitate Wraparound team meetings, ensuring to invite natural and formal supports, guide the family’s team through meaningful participation.
• Guide the Wraparound team through needs discovery, strengths identification, and family vision articulation.
• Lead development of Wraparound team agreements, team mission, the family vision statement, and development of the family’s goals.
• Carefully coordinate the development of crisis and safety plans that reflect family-driven, proactive, and reactive strategies.
• Update and review plans ensuring all team members are aligned and aware of responsibilities.
• Maintain ongoing communication and follow-up with all Wraparound and Child and Family Team members to ensure clear communication, accountability, and service delivery.
• Engage additional natural supports and professionals to expand the Child and Family team’s capacity.
• Support transition planning from the onset, guiding the family through benchmarks toward sustainable support and success.
• Incorporate tools such as the IP-CANS to inform planning and monitor progress.

Requirements

• One year experience in human services, preferably case management.
• Must qualify as a Medi-Cal Specialty Mental Health Services (SMHS) provider.
• Completion of the Statewide Standardized Child and Family Team (CFT) Facilitation Training.
• Valid CA driver’s license and proof of automobile insurance.
• Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.

Qualifications

• Bachelor’s Degree in social work, psychology, nursing, occupational therapy, or a related field from an accredited college or university preferred.
• Completion of the Statewide Standardized Child and Family Team (CFT) Facilitation Training.
• Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.
• Completion of the required training for Medi-Cal Specialty Mental Health Services (SMHS) provider.
