About the Role

Beginning July of 2026, all Behavioral Health Plans in California will be required to implement High-Fidelity Wraparound (HFW) to fidelity under the Full Service Partnership (FSP) program. All FSPs for children and youth must align with the principles, values, and standards of the HFW model. HFW provides a comprehensive, holistic, evidence-based, child/youth and family-driven process for responding when children and youth experience significant mental health or behavioral challenges, including those involved in multiple child/youth serving systems.

HFW is a team-based and family-centered evidence-based practice that includes an "anything necessary" approach to care for children and youth with the most intensive mental health or behavioral challenges. The HFW model combines a team-based case management and facilitation approach with individualized and community-based mental health services and supports tailored to meet the individualized needs of the child/youth and family.

Responsibilities

  • Actively participate in the engagement process by providing reassurance that the family’s circumstances and challenges will be handled confidentially.
  • Listen with empathy and without judgment to identify parents' and caregivers' needs, strengths, and key supports.
  • Help orient parents and family members to the Wraparound process, using lived experience to guide them through system structures, identify barriers, and assist in removing obstacles to success.
  • Address parents’ needs through empathetic communication, ensuring they feel supported, empowered, and equipped to advocate for themselves while being connected to meaningful resources.
  • Support parents in understanding the importance of involving and connecting with extended family and other supportive individuals to build permanent, lasting relationships and prepare for life beyond formal services.
  • Provide opportunities for parents to prepare before meetings and decision-making sessions so they can confidently advocate for themselves and their families.
  • Ensure that parents have the opportunity to share their perspectives during meetings and that their voices, cultures, and preferences are respected and reflected in the plans and strategies developed to support positive change.
  • Share observations and feedback when practices do not reflect fidelity to the core Wraparound principles and values (team-based, strengths-based, outcome-focused, persistent, and needs-driven).
  • Provide direct services to support the implementation of Child and Family Team (CFT) plan strategies and offer assistance during crisis situations as needed.
  • Support the development of effective parenting and self-care skills through one-on-one meetings and group sessions, sharing insights from lived experience.
  • Advocate within organizations and systems to ensure that policies and practices respect and honor the voice, vision, and lived experience of families as experts in their own lives and equal partners in their care.
  • Participate in evaluations, quality improvement activities, and ongoing training in alignment with Wraparound standards and best practices.

Requirements

  • High school diploma or GED.
  • Writing skills adequate to complete documentation as required.
  • Lived experience as a parent/caregiver (preferred).
  • Personal experience navigating the child welfare system, juvenile justice system, educational system, or mental health system for oneself or in a caregiver/parent capacity.
  • Personal or professional experience accessing community resources.
  • Commitment to practice cultural respectfulness and relevance.
  • Ability to maintain healthy boundaries while building trust.
  • Strong peer engagement and communication skills.
  • Medi-Cal Certified Peer Support Specialist (preferred). Must be willing to obtain and maintain certification after hire.
  • Valid CA driver’s license and proof of automobile insurance.
  • Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.

Preferred Experience

  • Training/experience in one or all of the following Evidenced Based Practices:
    • High Fidelity Wraparound (HFW)
    • Intensive Case Management (ICM)

  • Identify and recommend appropriate service types that align with client needs while staying within the bounds of training, expertise, and professional role.
  • Understand the range of services available within the organization and match those services to the specific needs and preferences of clients.

Crisis Intervention and Safety Planning

  • Handle crisis situations effectively, including suicidal ideation, self-harm, and acute psychiatric symptoms, by adhering to established safety protocols and accessing appropriate resources.
  • Participate as needed in the implementation of safety plans in collaboration with clients, their support systems, and the treatment team.

Productivity & Service Billing

  • Meet program productivity expectations on a monthly basis.

Core Competencies

  • Attendance and Reliability: Consistent and punctual attendance, dependable approach to fulfilling professional responsibilities, adhering to scheduled shifts, appointments, and meetings.
  • Collaboration and Teamwork: Work collaboratively with multidisciplinary teams, including psychiatrists, case managers, educators, outside agency providers, and other mental health professionals to coordinate care and provide integrated services.
  • Interpersonal Communication Skills: Establish rapport quickly, understand and address client needs, solicit and use input from team members and outside agencies, and facilitate communications among stakeholders. Communications are clear, concise, respectful, effective, and timely.
  • Quality of Care: Consistently deliver work and client care of exceptional quality while ensuring precision, thoroughness, and adherence to established standards and procedures.
  • Accuracy and Quality Improvement: Uphold status of a single signer (40% or lower return rate for three consecutive months) by completing accurate documentation with minimal errors. Actively participate in departmental and agency-wide quality improvement activities.
  • Efficiency, Time Management, and Organization: Organize and prioritize assignments to ensure urgent issues are resolved while not losing sight of longer-term goals. Submit accurate and complete progress notes within 3 days; crisis notes within 24 hours. Use collaborative documentation with clients regularly.
  • Cultural Competency: Awareness of personal cultural biases and assumptions, respect for the cultural backgrounds of clients, and tailoring interventions accordingly.
  • Professional Development: Commitment to continuous learning, skills improvement, and ability to adapt to changes in standards of practice in the field. Participate in both required and voluntary trainings.
  • Alignment to SCCS Principles and Values: Uphold integrity, respect others, maintain a positive attitude, and demonstrate empathy towards colleagues, clients, and customers.

Physical Requirements

  • Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
  • Senses of vision, hearing, and smell sufficient to regularly operate a motor vehicle, visualize threats in the environment or risks to clients, and operate safely in the field.
  • Required to occasionally lift and carry 10-20 pounds.
  • Regularly required to sit, stand, climb stairs (1 or more flights), and walk while in route to and during family visits.
  • Regularly required to sit and use a computer in office settings.

Pay

$22.00 - $22.00 per hour
