About the role
The Neville Center at Fresh Pond is a Joint Commission, American Healthcare Association Award-winning, short-term rehab, long term, and skilled nursing care community. We are conveniently located on the Cambridge/Belmont line and dedicated to our residents and team members.
Responsibilities
- Assess the health care needs of the resident, make a nursing diagnosis, collaborate with the nursing team to develop an interdisciplinary plan of care utilizing all available resources.
- Communicate the plan of care to all care providers to ensure continuity of care for residents.
- Establish and maintain functional communications with staff, other disciplines, residents, families, medical staff, and the community.
- Absorb and apply knowledge to assess the effects of medications and treatments, and monitor these effects to provide appropriate feedback to the interdisciplinary team and medical managers.
- Lead and direct the nursing staff in the safe, effective delivery of care by providing a structured approach.
- Provide education to residents regarding their healthcare issues and care plan strategies.
- Knowledge and ability regarding infection control, fire and disaster policies, and other facility and/or state and federal policies and practices that ensure a safe environment for residents, community, and staff.
- Knowledge of and ability to protect resident rights, treat residents, staff, and community with respect and dignity, and handle resident and facility property and information with honesty, confidentiality, and respect.
Requirements
- Predictable schedule with weekend rotation required.
- Full-time RN bonus of $6,000, prorated for part-time.
- LPN bonus of $2,500, prorated for part-time.
Qualifications
- Registered Nurses (RN): Bachelor's degree in Nursing or related field, current RN license, and one year of relevant experience.
- Licensed Practical Nurses (LPN): High school diploma or equivalent, current LPN license, and one year of relevant experience.
Skills
- Strong communication skills.
- Ability to work independently and as part of a team.
- Knowledge of electronic health records (PCC preferred).
- Experience with infection control and fire/disaster policies.

Pay
- Hourly rate ranges from $40 (starting LPN) to $60 per hour (experienced RN with shift differential).
