About the role
Oversee patient recovery at home by providing clinical services including assessments, medication management, and patient/family education.
Responsibilities
- Coordinate, implement, and evaluate individual home care plans
- Review insurance information and communicate findings with caregivers
- Supervise and mentor peers and new nurses during field visits
- Ensure comprehensive documentation and effective communication
Requirements
- Graduate from an accredited Nursing School; BSN preferred
- Minimum of 3 years homecare experience; supervisory experience preferred
- Valid NYS RN license, driver’s license, and car
- Knowledge of Home Health regulations and Oasis Certification preferred
Skills
- Strong communication, time management, and documentation skills
- Proficiency in EMR and Word
