Nurse Team Leader

About the role

This role involves overseeing advanced clinical home care services, including patient assessment, medication management, and family education, to support patients recovering at home. As a primary coordinator, you will evaluate patient needs, review insurance information, and ensure effective communication among family and professional caregivers. You will utilize your strong communication, documentation, and time management skills to provide high-quality care with autonomy and respect for your expertise. Additional responsibilities include precepting new nurses and supervising peers during field visits.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred
• Minimum of 3 years homecare experience; supervisory/leadership preferred
• NYS RN license
• Driver license and car required
• Oasis Certification preferred
• Knowledge of Home Health regulations
• Proficiency in EMR and Word applications
