About the role
Provide advanced clinical home care services, including patient assessment, medication management, and family education.
Responsibilities
- Coordinate and evaluate patient recovery plans
- Review insurance details and communicate findings with family and caregivers
- Supervise and mentor nurses during field visits
- Precept new staff
- Maintain high standards of documentation, communication, and time management
- Ensure optimal patient care with autonomy and respect for opinions
Requirements
- Graduate of an accredited Nursing School (BSN preferred)
- At least 3 years of homecare experience
- Current NY RN license
- Valid driver’s license and car required
- Oasis certification and knowledge of Home Health regulations preferred
- Strong supervisory, mentoring, and coaching abilities
- Proficiency in EMR and Microsoft Word
