We are seeking a Neighborhood Nurse of high character who exhibits compassion and understanding, with a positive attitude and a commitment to advocating for residents with varying degrees of disability.
Requirements
- Current RN or LPN license to practice nursing in the State of Delaware
- CPR (BLS) certification
- Ability and willingness to actively supervise and assess Resident Care Technicians (CNAs)
- Proficiency in lifting and transferring adults, including use of lift devices
- Strong verbal and written communication skills
- Ability to utilize technology tools for resident health reporting
- High school diploma or equivalent
- At least 1 year of nursing experience (preferred)
- Willingness to undergo background check, TB test, and drug test prior to starting
Responsibilities
- Lead and direct Resident Care Technicians (RCTs) to ensure healthcare activities meet MCC standards
- Assist residents with personal hygiene and self-care, including bathing, feeding, toileting, and dressing
- Administer medications accurately and provide treatments as ordered
- Perform and record routine assessments of residents' physical and emotional health
- Monitor and treat residents' acute and chronic medical conditions
- Utilize the nursing process to identify health problems, plan individualized care, implement interventions, and measure outcomes
- Communicate changes in residents' conditions promptly to the Director of Nursing (DON), Assistant Director of Nursing (ADON), Neighborhood Nurse Managers, Nursing Supervisor, and Medical Director
- Report changes in residents' medical conditions to their physicians as per procedure
- Communicate with residents and their families regarding healthcare plans
- Evaluate, mentor, coach, counsel, and discipline RCTs as needed
- Participate actively in the Interdisciplinary Team (ID Team) for assigned residents
- Provide care and services for individuals in the Respite Care Program
- Complete mandatory in-services and attend staff meetings
- Perform other duties as assigned
