DaVita Integrated Kidney Care (IKC) is looking for a Full-Time Nurse Practitioner (NP) to support patients in Stamford, South Norwalk, and New Haven, CT, as well as Yonkers and White Plains, NY. Must live locally.

About the role

As an IKC Nurse Practitioner (NP), you will support complex patients by delivering holistic, integrated care to improve their quality of life. The NP provides comprehensive healthcare—both in-person and via telehealth—across the many co-morbid chronic conditions common to CKD/ESKD patients within a designated market or geographical region. The role functions autonomously while collaborating with the healthcare team to provide, coordinate, and facilitate quality, cost-effective care. The NP assumes a clinical leadership role, coordinating activities to meet patients' health needs.

Responsibilities

  • Perform enhanced annual wellness visits or Comprehensive Health Evaluations (CHEs) to identify and evaluate patients’ co-morbid conditions and assist with medical, social, emotional, and financial concerns through medical record review, consultation with patients, families, and healthcare team members.
  • Manage complex patients, focusing on reducing hospitalizations and improving health outcomes.
  • Manage care for CKD/ESKD patients, delaying disease progression, reducing medical costs/utilization, and preventing/reducing hospitalizations through proactive post-discharge planning and follow-up.
  • Leverage prescriptive authority, medication management, ordering/interpretation of labs/diagnostics, and collaboration with other providers.

Requirements

  • Passion for delivering care to patients with complex, chronic illnesses.
  • Minimum of two (2) years' experience as a Nurse Practitioner (NP).
  • Current unrestricted NP license to practice in CT; license in NY or ability to obtain after hire.
  • Current Drug Enforcement Administration (DEA) license or eligibility to obtain within 90 days of hire.
  • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
  • Valid driver’s license and insurance within the state of residence.
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook.
  • Demonstrated proficiency in clinical assessment and management of complex comorbidities, diagnosis, planning, implementation, documentation, and evaluation of chronic disease care.
  • Strong clinical judgment and confidence in autonomous decision-making within established protocols and evidence-based guidelines.
  • Home office, high-speed internet, and reliable transportation required.

Preferred Qualifications

  • Experience working in Value Based Care or Population Health, including documenting and coding from a Medicare Advantage (MA) risk perspective and conducting Health Risk Assessments on high-risk/chronically ill patients.
  • 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology, or Case Management.
  • Experience managing population health (hospitalization reduction).
  • Prior experience with Cerner or similar EHR systems.

Schedule

  • Monday–Friday schedule with flexibility to accommodate patient and nephrology partner availability.
  • Full-time, self-managed schedule.
  • No nights, weekends, or on-call shifts.

Pay

The salary range for this role is $118,000.00 – $135,000.00 per year. Compensation depends on qualifications, skills, competencies, and experience. DaVita offers a competitive total rewards package, including a 401(k) match, healthcare coverage, and a broad range of benefits.
