About the role

The IKC Nurse Practitioner (NP) supports complex patients navigating a challenging healthcare system, providing holistic, integrated care.

Responsibilities

  • Supports patients with co-morbid conditions through enhanced annual wellness visits and Comprehensive Health Evaluations (CHEs).
  • Manages complex patients to reduce hospitalizations and improve health outcomes.
  • Coordinates care for CKD/ESKD patients, focusing on delaying disease progression and managing medical costs.
  • Leverages prescriptive authority, medication management, and collaboration with other providers.
  • Provides in-person and telehealth care, with a Monday-Friday schedule accommodating patient and nephrology partner's availability.
  • Assumes a clinical leadership role, coordinating with the healthcare team and healthcare systems.

Requirements

  • Minimum of two (2) years' experience as a Nurse Practitioner (NP).
  • Current unrestricted NP license to practice in the state of CT, NY, or eligibility to obtain within 90 days of hire.
  • Current DEA license required or eligible to obtain within 90 days of hire.
  • Current CPR or BLS certification.
  • Intermediate computer skills and proficiency in MS Word, Excel & Outlook.
  • Demonstrated proficiency in clinical assessment and management of complex comorbidities.
  • Strong clinical judgment and confidence in autonomous decision-making.
  • Preferred experience in Value-Based Care or Population Health, including documentation and coding from a MA perspective.
  • Experience with Cerner or similar EHR system preferred.

Pay

$118,000.00 - $135,000.00/year

Schedule

Monday-Friday schedule with flexibility to accommodate patient and nephrology partner's availability.
