About the role
The IKC Nurse Practitioner (NP) supports complex patients, assisting them in navigating a challenging healthcare system. They deliver holistic, integrated care to improve the quality of life for kidney patients, focusing on delaying disease progression, managing medical costs, and preventing hospitalizations.
Responsibilities
- Supports patients with complex, chronic illnesses through enhanced annual wellness visits and Comprehensive Health Evaluations (CHEs).
- Manages the care of CKD/ESKD patients, focusing on reducing hospitalizations and improving health outcomes.
- Leverages prescriptive authority, medication management, ordering/interpreting labs/diagnostics, and collaborating with other providers.
- Provides care both in-person and via telehealth, coordinating with the healthcare team and healthcare systems to meet patients' health needs.
- Assists with patient and nephrology partners' availability, accommodating their schedules.
- Works full-time, self-managed schedule with flexibility in hours.
- Performs clinical assessments and management of complex comorbidities, ensuring quality patient care.
- Leverages experience in Value-Based Care or Population Health, including documenting and coding from a Medicare Advantage risk perspective.
Requirements
- Minimum of two (2) years' experience as a Nurse Practitioner (NP).
- Current unrestricted NP license to practice in Connecticut - License in New York or eligibility to obtain within 90 days of hire.
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook.
- Demonstrated proficiency in clinical assessment and management of complex comorbidities, diagnosis, planning, implementation, documentation, and evaluation.
- Strong clinical judgment and confidence in autonomous decision-making while practicing within established protocols and evidence-based guidelines.
- Preferred experience working in Value Based Care or Population Health, conducting Health Risk Assessments on high-risk/chronically ill patients.
- Experience in Internal Medicine, Cardiology, Nephrology, or Case Management.
- Experience managing population health (hospitalization).
- Experience with Cerner or similar EHR system.
