About the role
The IKC Nurse Practitioner (NP) supports complex patients navigating a challenging healthcare system, providing holistic, integrated care.
Responsibilities
- Supports patients with co-morbid conditions through medical, social, emotional, and financial needs.
- Provides comprehensive care, both in-person and via telehealth, for CKD/ESKD patients.
- Manages complex patients to reduce hospitalizations and improve health outcomes.
- Assists with post-discharge planning and follow-up for high-risk patients.
- Leverages prescriptive authority, medication management, and collaboration with other providers.
- Works within established protocols and evidence-based guidelines.
- Manages population health and documents and codes from a Medicare Advantage risk perspective.
Requirements
- Minimum of two (2) years' experience as a Nurse Practitioner (NP).
- Current unrestricted NP license to practice in the state of CT.
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook.
- Demonstrated proficiency in clinical assessment and management of complex comorbidities.
- Strong clinical judgment and confidence in autonomous decision-making.
- Preferred experience working in Value Based Care or Population Health.
- Preferred experience with Cerner or similar EHR system.

- Intermediate computer skills and proficiency in MS Word, Excel & Outlook.
- Demonstrated proficiency in clinical assessment and management of complex comorbidities.
- Strong clinical judgment and confidence in autonomous decision-making.
- Preferred experience working in Value Based Care or Population Health.
- Preferred experience with Cerner or similar EHR system.

Pay
$118,000.00 - $135,000.00/year
Schedule
Monday- Friday schedule with the ability to accommodate patient and nephrology partner’s availability. Self-managed schedule, No nights, weekends and on-call!
