About the Role

Provide comprehensive care for patients with acute and chronic illnesses, primarily older adults. Take full accountability for patient outcomes, including assessment, diagnosis, treatment, management, education, and care coordination.

Responsibilities

  • Lead and collaborate with care teams, including care promoters, facilitators, and coordinators.
  • Engage with hospital and transitional care teams for patients in various settings like hospital, SNF, LTC, or homebound.
  • Monitor key metrics such as patient admissions, patient experience, clinical gaps, and costs, working towards organizational goals.
  • Support center management, participate in clinical meetings, risk and quality programs, and foster a positive, accountable culture.
  • Supervise or collaborate with Advanced Practice Practitioners, ensuring quality and compliance.

Requirements

  • Current Nurse Practitioner certification and Basic Cardiac Life Support certification.
  • Board certification preferred; DEA number may be required depending on state.
  • Minimum one year of clinical experience; strong new graduates considered.
  • Excellent communication skills, availability, and accessibility for patients.
  • Proficiency in evidence-based medicine, leadership, quality improvement, and self-care.
  • Ability to exercise independent judgment, work collaboratively, and travel locally or nationally as needed.
