About the role

The Primary Care Provider (PCP) Nurse Practitioner delivers comprehensive care to older adults, managing acute and chronic conditions while promoting health and wellness.

Responsibilities

• Provide independent, high-quality clinical care and outcomes accountability for a patient panel of up to 450 individuals.
• Act as a health coach by setting and following up on patient health goals, and simplifying actions for better behavior change.
• Collaborate with care teams, specialists, hospitalists, and transitional care teams to coordinate patient care across settings.
• Lead and supervise care teams, including medical assistants, care facilitators, and coordinators.
• Participate in practice management, patient recruitment, and culture-building activities within the center.
• Ensure compliance with regulatory standards, quality metrics, and company policies.

Qualifications

• Current Nurse Practitioner certification and Basic Cardiac Life Support (BCLS) certification.
• Board certification by AANP or ANCC preferred; DEA license may be required.
• Minimum one year of clinical experience preferred; strong new graduates considered.
• Excellent communication skills, proficiency in Microsoft Office, and ability to travel locally and nationally as needed.
