About the Role

The Primary Care Provider (PCP) Nurse Practitioner offers comprehensive care to older adult patients, managing acute and chronic conditions with accountability for patient outcomes.

Responsibilities

• Independently assess, diagnose, treat, and coordinate care for patients
• Lead care teams and collaborate with hospitalists and specialists
• Engage in health coaching and patient education
• Participate in practice management and quality improvement initiatives

Requirements

• Current Nurse Practitioner certification
• Basic Cardiac Life Support (BCLS) certification
• Board certification preferred
• Strong clinical judgment, leadership, and communication skills
• Proficiency in evidence-based medicine and patient relationship building
• Ability to collaborate effectively within care teams
• Local travel required
• Fluency in English

Prior clinical experience preferred; new graduates will be considered. Must exercise independent judgment and adhere to compliance standards.

Pay

Salary range from $111,140 to $158,771, with potential bonuses.
