Nurse Practitioner / Physician Assistant (Family Medicine — Float)

About the role

This position is for a Nurse Practitioner or Physician Assistant specializing in Family Medicine, operating as a float across multiple locations in Trenton, NJ. The role is full-time and requires flexibility to work across various clinical settings, including school-based health sites and a mobile health unit, with a focus on serving uninsured and underserved populations.

Responsibilities

• Adapt to various care settings and team environments
• Provide comprehensive care to patients of all ages, including adults, children, and women’s health
• Collaborate with multidisciplinary teams to ensure patient-centered care
• Conduct assessments, diagnose conditions, and develop treatment plans
• Perform physical exams and order diagnostic tests
• Prescribe medications and manage chronic conditions
• Engage in preventive care and health education
• Participate in quality improvement initiatives and ongoing professional development

Requirements

• Master's degree in Nursing or completion of an accredited PA program
• Active NJ NP or PA license with current CDS and DEA
• Minimum of 2 years clinical experience
• Experience in community health or multi-site outpatient settings preferred
• Spanish bilingual preferred
• Reliable transportation within the Trenton area

Qualifications

• NP board certification (AANP or ANCC) or NCCPA certification for PAs
• Existing Medicaid, Medicare, and MCO credentialing preferred
