Nurse Practitioner / Physician Assistant

Pine Park Health is looking for a Full-Time Nurse Practitioner / Physician Assistant responsible for delivering care in our East Bay San Francisco market (Oakland, Walnut Creek, Concord). What you'll do
- Provide onsite primary care services to older adults living in Senior Living Communities
- Develop and execute detailed person-centered care plans on all primary care patients
- Work with Medical Director to identify and develop policies to address issues that arise which affect day-to-day practice, culture or clinical guidelines, including for emerging disease(s)
- Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation
- Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
Qualifications
- Geriatrics experience preferred
- Comfortable with a fast-paced and changing environment
- Willingness to provide onsite clinical care across the East Bay and San Francisco region (Concord, Walnut Creek, Oakland)
- Desire to work with complex care patients and support the building of a holistic care model
- Strong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical team
- Strong written and verbal communicator
- Ability to work with tech-enabled care delivery tools (e.g., project management, 'checklist' management, remote monitoring, etc.) used together to improve patient care and team function
- This role involves driving between partner communities as needed

Pay
The base salary range for this role is $145,000 to $165,000, PLUS a performance bonus paid quarterly. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
