This position covers all zip codes in Kitsap County, WA. ComplexCare Solutions (CCS) specializes in delivering high‑value home and virtual assessments for health plans nationwide. The company focuses on member engagement to evaluate health status, gaps in care, potential risks, and care‑management opportunities to drive better clinical outcomes. On behalf of SME Medical Delaware, P.A., CCS is seeking Advanced Practice Providers passionate about helping local communities. Providers will spend a full hour with each member to capture a comprehensive picture of personal needs and determine how to keep members safely at home.
Pay
Compensation Pay Range: $2,400.00 – $10,000.00 per month (potential income). Earnings vary based on completed assessments, state of residence, and business needs; there is no guarantee of visits or minimum income. In‑home visit rate: $120 – $140 depending on state of residence. Telehealth rate (when available): $85 per completed assessment.

Requirements
- Active, unencumbered license to practice nursing.
- ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist in Family, Adult, Gerontology, or Emergency Medicine.
- Current CPR certification.
- Compliance with required healthcare‑personnel vaccinations and testing: Hepatitis B, Influenza, MMR, Pertussis, Tetanus/Diphtheria (Tdap), Varicella, Tuberculosis.
Qualifications
- Home health experience (preferred).
- Understanding of Medicare, Medicaid, and health‑plan benefit structures.
- Knowledge of CMS regulations, NCQA HEDIS guidelines, and evidence‑based clinical practice standards (ADA, ACC, AHA).
- Familiarity with ICD‑10 and CPT‑4 coding practices.
- Ability to practice autonomously in a remote clinical environment, including independent patient assessments and evidence‑based treatment planning.

- Effective communication with elderly and chronically ill patients and their families.
- Excellent customer‑service skills and ability to multitask.
- Bi‑lingual or multi‑lingual abilities (a plus).
