Nurse Practitioner

Full-Time Exempt position with a salary range of $90,000 - $130,000 annually.

CNY Family Care is a Primary Care practice and a Patient Centered Medical Home, taking a team-based approach to medicine. Our Nurse Practitioners have ready access and ability to precept cases with our Physicians in a supportive, collaborative environment. Responsibilities

• Provide and coordinate health care services to patients and their families
• Promote maintenance of health, prevention of illness, and stability
• Provide care during acute and chronic phases of illness
• Guide and counsel individuals and families
• Refer patients to other health care providers as appropriate
• Record and update patient EMR in accordance with HIPAA and practice standards and procedures

Requirements

• Current, certified NYS Nurse Practitioner license
• Minimum of two (2) years’ experience in a family practice or Urgent Care setting is desired (not required)
• Previous experience working with Medent (EMR) is a plus
• Excellent working knowledge of chronic disease states and strong critical thinking skills
• Strong interpersonal and communication skills
