Nurse Practitioner or Physician Assistant - PRN

About the role

Seeking a provider role in in-home health assessments, you will conduct comprehensive evaluations of patients in their homes or facilities, lasting up to one hour.

Responsibilities

• Reviewing medical history, medication adherence, social factors, and risks
• Performing and ordering tests
• Collaborating with physicians, case managers, social workers, and caregivers
• Documenting assessments per guidelines
• Help identify health issues, improve care quality, and close care gaps

Qualifications

• Master's Degree or equivalent experience with NP licensure
• Current NP licensure with prescriptive authority in the practice state
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care
• Current BLS, ACLS, or CPR certification
• At least 1 year of NP experience preferred; new graduates encouraged to apply
• Valid driver's license and auto insurance with required coverage
• Strong computer skills and excellent communication abilities
