Nurse Practitioner or Physician Assistant - PRN

About the Role

This position involves conducting comprehensive in-home health assessments for adult and geriatric patients. Providers review medical history, medications, social factors, and health risks to improve care quality and identify unrecognized conditions. Assessments are delivered at members' residences, including homes and Skilled Nursing Facilities.

Responsibilities

• Perform in-home health assessments, health education, and referrals
• Administer point-of-care and lab tests
• Coordinate with physicians, case managers, and family members
• Follow up on care plans and document visits accurately and thoroughly per protocols

Requirements

• Master’s Degree or equivalent with NP licensure and prescriptive authority in the state
• Board certification in relevant specialty (Adult, Gerontology, Family, Emergency, or Acute Care)
• Current BLS, ACLS, or CPR certification
• Strong communication and computer skills
• Valid driver’s license with insurance coverage

Preferred Qualifications

• 1+ year NP experience (new graduates are encouraged to apply)
