Nurse Practitioner or Physician Assistant - PRN

About the role

Join a healthcare provider specializing in in-home health assessments for adult and geriatric populations. You will conduct comprehensive evaluations during visits that assess health, medical history, medication adherence, social factors, and risks. This role involves collaborating with physicians, case managers, social workers, and caregivers.

Responsibilities

• Perform in-home health assessments, including comprehensive evaluations of health, medical history, medication adherence, social factors, and risks.
• Order tests and document findings accurately.
• Provide patient education and follow-up care at members' residences, including homes and skilled nursing facilities.

Requirements

• Master’s Degree or equivalent in a relevant field.
• Current NP licensure with prescriptive authority.
• Certification in adult or family care specialties.
• BLS/ACLS/CPR certification.
• Valid driver’s license and appropriate auto insurance.
• Strong communication and computer skills.

Experience is preferred, but new graduates are encouraged to apply.
