Nurse Practitioner or Physician Assistant, Full-Time

As an Advanced Practice Provider at Theoria Medical, you’ll care for patients over time—building trust, strengthening continuity, and making a lasting difference for vulnerable populations. Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
- Communicate with facility nursing staff via phone or secure messaging as needed
- Maintain reliable on-site attendance for scheduled shifts
- Ensure compliance with clinical standards, facility policies, and regulatory requirements
Qualifications
- Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants)
- Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine
- Ability to perform essential functions of the position satisfactorily, with or without reasonable accommodation
Pay
The estimated compensation range for this position is $104,000 to $169,000 per year, based on a productivity-based model. This role is eligible for a longevity bonus of up to $50,000 and additional quality bonus programs. Sign-on bonuses and relocation assistance may be available for eligible candidates.
Schedule
- 6-hour facility shifts for full-time positions (rounds generally start between 7–10 a.m.)
- No on-call or overnight requirements
