Seeking a provider to conduct comprehensive in-home health assessments for adult and geriatric members, lasting up to one hour.
Responsibilities
- Review medical history, medication adherence, and social factors
- Administer tests and perform assessments including medical history, diagnosis, treatment, and health education
- Order and administer point-of-care and lab tests
- Collaborate with physicians, case managers, social workers, and caregivers
- Document findings and follow up with primary care physicians
- Provide patient education
- Work primarily in members' residences, including homes and Skilled Nursing Facilities
Qualifications
- Master’s degree or equivalent experience with NP or PA licensure
- Current licensure with prescriptive authority and certification in relevant specialties
- Valid CPR/BLS/ACLS certifications
- Minimum of one year NP or PA experience
- Ability to travel extensively, including overnight stays
- Valid driver’s license and insurance
- Strong computer and communication skills
