Nurse Practitioner or Physician Assistant - Full Time

About the role

Join a national in-home clinical network as a provider conducting comprehensive health assessments for adult and geriatric members in their homes or Skilled Nursing Facilities.

Responsibilities

• Performing assessments covering medical history, diagnosis, treatment, social factors, and risks
• Administering and ordering point-of-care and lab tests
• Collaborating with physicians, case managers, social workers, and caregivers
• Providing patient education and follow-up in coordination with primary care providers
• Documenting all activities according to protocols

Qualifications

• Master's Degree or equivalent experience with NP or PA licensure, current in-state licensure with prescriptive authority
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care
• Minimum one year of NP or PA experience
• Valid driver's license, automobile insurance, and ability to travel extensively
• Strong communication, computer skills, and familiarity with medical records software
