Nurse Practitioner, In-Home Visits

About the Role

We are seeking a part-time In-Home Nurse Practitioner (NP) to conduct health evaluations for Medicare Advantage, Medicaid, and commercial plan members in their home environment. The role involves performing pre-scheduled in-home visits within a designated metro area, reviewing medical histories, conducting physical assessments, ordering diagnostic tests as needed, providing patient education, and closing care gaps while ensuring safety and quality.

Responsibilities

• Perform pre-scheduled in-home health evaluations for plan members
• Review and document medical histories
• Conduct comprehensive physical assessments
• Order and interpret diagnostic tests as clinically indicated
• Provide patient education and support
• Identify and close care gaps
• Ensure safety and quality of care in the home setting
• Travel locally within the designated metro area

Requirements

• Active state Nurse Practitioner license
• Board certification as a Nurse Practitioner
• Multi-state licensure preferred
• Experience with in-home health evaluations preferred
• Flexibility to travel locally for patient visits

Pay

Compensation includes hourly pay plus performance-based incentives.
