Nurse Practitioner, In-Home Visits

Role Overview

Seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations, fostering trusted relationships and promoting better health outcomes.

Responsibilities

• Perform 45-minute personalized health assessments, including medical history review, physical evaluation, and diagnostic tests as ordered.
• Utilize an iPad and clinical workflow for evaluations, without prescribing or altering treatments.
• Engage with patients both in-person and virtually, providing comprehensive preventive services and health education.
• Travel locally and within licensed states as needed.

Qualifications & Skills

• Active, unrestricted licensure with board certification.
• PREFERRED: Multi-state licenses.
• Ability to cover primary locations, such as Hartford, CT.
