About the Role

Part-time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to help bridge gaps in care and improve health outcomes.

Responsibilities

• Conduct 45-minute visits including medication and medical history review, physical evaluation, and optional diagnostic tests.
• Utilize an iPad and clinical workflow; no prescribing or treatment changes required.
• Engage with patients, build trust, and provide personalized health assessments both in-person and virtually.
• Adapt to various home settings, including environments with pets or allergens.
• Provide quality care aligned with patient needs and safety standards.

Requirements

• Active, unrestricted licenses with board certification; multi-state licenses preferred.
• Ability to travel locally and work in diverse home environments.
