About the Role

A Part-Time Clinician (Nurse Practitioner or Physician Assistant) will provide in-home health evaluations to bridge gaps in care and promote better health outcomes. The role involves conducting 45-minute visits that include medical history reviews, physical evaluations, and optional diagnostic tests, all managed via an iPad workflow. Emphasis is placed on building trusted relationships, connecting patients to appropriate care, and may include local travel and virtual visits.

Responsibilities

• Engage with patients in their homes to conduct comprehensive health evaluations.
• Perform medical history reviews, physical evaluations, and optional diagnostic tests.
• Utilize an iPad to document and manage patient workflows.
• Build trusted relationships with patients to promote better health outcomes.
• Connect patients to appropriate care resources and services.
• Maintain flexibility for local travel and virtual visits as needed.

Requirements

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification in your specialty.
• Multi-state licenses preferred.
• Ability to cover primary locations, particularly Hartford, CT.

Pay

Competitive pay range of $46.03 - $99.14 per hour.
