Nurse Practitioner - Healthcare Provider

Role Overview

Seeking a Nurse Practitioner to provide in-home, comprehensive clinical assessments and manage patients with acute or chronic conditions within their residences. The role involves conducting health evaluations, managing care during medical or rehabilitation episodes, and collaborating with an interdisciplinary team. You will educate patients and caregivers, perform evidence-based screenings, and ensure accurate documentation, including ICD-10 coding. Key Responsibilities

• Perform in-home health assessments and screenings
• Manage acute and chronic medical issues, including diagnostics and interventions
• Coordinate care with the interdisciplinary team
• Conduct advanced care planning and provide education to patients and caregivers
• Ensure complete and accurate documentation of patient encounters

Qualifications & Skills

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in Colorado (or ability to obtain)
• Valid driver's license and reliable transportation
• Current DEA licensure or ability to acquire
• Preferred: 1+ years clinical experience, familiarity with geriatric or home care, proficiency with EMRs
