Role Overview Seeking a Nurse Practitioner to provide in-home, comprehensive clinical assessments and manage patients with acute or chronic conditions within their residences. The role involves conducting health evaluations, managing care during medical or rehabilitation episodes, and collaborating with an interdisciplinary team. You will educate patients and caregivers, perform evidence-based screenings, and ensure accurate documentation, including ICD-10 coding.
