Nurse Practitioner - Healthcare at Home

About the Role

This position involves providing in-home clinical care to high-risk patients, primarily in Buchanan and Andrew counties, without night, weekend, or holiday shifts.

Responsibilities

• Perform in-home medical evaluations, including history, vital signs, and screenings
• Manage patients during acute episodes or exacerbations of chronic illnesses
• Coordinate with interdisciplinary care teams and document clinical findings accurately
• Engage in advanced care planning and provide counseling to patients and caregivers

Requirements

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure in Missouri and valid driver's license with reliable transportation
• Ability to navigate varied environments and perform physical duties as needed

Preferred Qualifications

• Clinical experience in geriatrics, home health, or long-term care
• Experience with complex behavioral, social, or functional patient needs
• Proficiency with electronic medical records and healthcare technology
