About the Role
This position involves providing in-home medical care within specific counties, supporting patients' physical, mental, and social health needs. It requires daily travel to patient homes, collaborating with an interdisciplinary team to develop and update personalized care plans, perform basic nursing procedures, and educate patients and their families on health management. The role emphasizes preventive care, reducing hospital admissions, and ensuring high-quality, holistic patient support.
Responsibilities
- Conduct initial and ongoing assessments of patients
- Develop, review, and modify care plans
- Perform basic nursing procedures (e.g., IVs, blood draws, wound care)
- Coordinate with clinical teams and follow post-discharge protocols
- Educate patients and families to promote wellness and disease management
- Participate in quality improvement initiatives and stay current with medical research
Requirements
- Current RN or NP license, with certification from AANP or ANCC
- Prescriptive authority and DEA licensure or ability to obtain
- Reliable transportation and valid driver’s license
- Preferred: 2+ years of clinical NP experience in primary or home health care
- Strong organizational, communication, and computer skills
Pay
Compensation ranges from $109,500 to $164,000, including base pay and bonuses.
