Nurse Practitioner - Care at Home

About the Role

Provide in-home care to high-risk patients, conducting comprehensive assessments, managing acute and chronic conditions, and facilitating care planning. Collaborate with interdisciplinary teams to deliver preventive and personalized care, ensuring accurate documentation and patient education.

Responsibilities

• Perform medical history reviews, physical exams, screenings, and point-of-care testing
• Manage acute episodes and exacerbations of chronic illnesses
• Support short-term rehabilitative needs
• Communicate with care teams and document patient information thoroughly
• Engage in advanced care planning and patient counseling

Qualifications & Skills

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Unrestricted licensure and reliable transportation
• Ability to navigate diverse environments and perform clinical duties

Preferred Experience

• 1+ years clinical experience, especially in geriatrics, home health, or long-term care
• Experience with complex behavioral, social, or functional needs
• Familiarity with electronic medical records and end-of-life discussions
