Nurse Practitioner - Care at Home

About the Role

This position involves providing in-home clinical care to high-risk patients in Buchanan and Andrew counties, emphasizing comprehensive assessments, managing acute and chronic conditions, and supporting short-term rehabilitation episodes.

Responsibilities

• Perform in-home assessments including medical history, medication reconciliation, vital signs, and physical exams.
• Manage patients during acute episodes or chronic condition exacerbations with testing, diagnostics, and interventions.
• Document thoroughly with ICD-10 coding and collaborate with interdisciplinary care teams.
• Conduct advanced care planning conversations and provide education and counseling to patients and caregivers.

Requirements

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted licensure in Missouri (or ability to obtain) and a valid driver’s license with reliable transportation.
• Ability to navigate various environments and perform physical tasks as needed.

Preferred Qualifications

• Clinical experience in geriatrics, home health, or long-term care.
• Familiarity with complex behavioral and social needs.
• Proficiency with electronic medical records.
