About the role

We are seeking a dedicated Full-Time Nurse Practitioner to provide primary care to elderly participants in a community-based program.

Responsibilities

  • Perform physical assessments and develop individualized care plans.
  • Evaluate participant complaints and administer appropriate treatments.
  • Conduct semi-annual reassessments and in-person evaluations for new enrollees.
  • Collaborate with an Interdisciplinary Team to ensure comprehensive, coordinated care.
  • Provide education and guidance to participants and caregivers.
  • Oversee residents in skilled nursing facilities and coordinate with primary care providers.
  • Participate in discharge planning and manage emergent and acute needs.

Requirements

  • Licensed Registered Nurse (RN) with current state licensure.
  • Graduate of an accredited nursing program.
  • Certified as a Nurse Practitioner by a recognized certifying organization.
  • At least one year of experience working with the elderly population.
  • Training or related experience in the role.

Preferred Qualifications

  • Geriatric Nurse Practitioner certification with prescriptive authority.
  • Two or more years of geriatric NP experience.

This Role Requires

  • Providing care to elderly residents, including evaluation and treatment.
  • Coordination with healthcare team members and discharge planning.
  • Intermittent local travel to participants’ homes.
