About the role
We are seeking a dedicated Full-Time Nurse Practitioner to provide primary care to elderly participants in a community-based program.
Responsibilities
- Perform physical assessments and develop individualized care plans.
- Evaluate participant complaints and administer appropriate treatments.
- Conduct semi-annual reassessments and in-person evaluations for new enrollees.
- Collaborate with an Interdisciplinary Team to ensure comprehensive, coordinated care.
- Provide education and guidance to participants and caregivers.
- Oversee residents in skilled nursing facilities and coordinate with primary care providers.
- Participate in discharge planning and manage emergent and acute needs.
Requirements
- Licensed Registered Nurse (RN) with current state licensure.
- Graduate of an accredited nursing program.
- Certified as a Nurse Practitioner by a recognized certifying organization.
- At least one year of experience working with the elderly population.
- Training or related experience in the role.
Preferred Qualifications
- Geriatric Nurse Practitioner certification with prescriptive authority.
- Two or more years of geriatric NP experience.
This Role Requires
- Providing care to elderly residents, including evaluation and treatment.
- Coordination with healthcare team members and discharge planning.
- Intermittent local travel to participants’ homes.
