About the role
Provide primary care to elderly participants in a community-based program.
Responsibilities
- Perform physical assessments
- Develop and implement care plans
- Evaluate complaints and provide treatment
- Educate participants and caregivers
- Collaborate with an interdisciplinary team
- Oversee residents in skilled nursing facilities
- Coordinate care with physicians
- Conduct comprehensive history and physicals
- Participate in discharge planning
Requirements
- Licensed RN and certified Nurse Practitioner
- At least one year of experience working with the frail elderly
- Geriatric Nurse Practitioner certification preferred
- Additional geriatric experience preferred
Skills
- Strong assessment and treatment skills
- Excellent communication skills
- Knowledge of elder care standards
Additional Information
