Responsibilities
- Round on patients daily, building real continuity of care
- Diagnose and manage a wide range of acute and chronic conditions
- Work hand-in-hand with interdisciplinary care teams and facility leadership
- Chart efficiently using AI-integrated EHR documentation
Requirements
- Active or eligible NM NP or PA-C license
- DEA and national certification (or ability to obtain)
- SNF experience is a plus, but not required — new grads are welcome
