Nurse Practitioner (Full-Time)

About the Role

We are seeking a Full-Time Nurse Practitioner to provide primary care for frail elderly participants under the supervision of the Medical Director. You will work as part of an Interdisciplinary Team to coordinate care, perform comprehensive history and physical exams, and participate in discharge planning. The role includes providing oversight and medical care for residents of Skilled Nursing Facilities, collaborating with primary care physicians and medical directors.

Responsibilities

• Conduct physical assessments of frail elderly participants
• Develop and implement individualized care plans
• Evaluate participant complaints and adjust care as needed
• Provide education to participants and caregivers
• Perform comprehensive history and physical exams
• Coordinate care as part of an Interdisciplinary Team
• Participate in discharge planning
• Provide medical oversight and care for residents of Skilled Nursing Facilities
• Collaborate with primary care physicians and medical directors

Requirements

• Valid RN license
• Nurse Practitioner certification
• At least one year of experience with elderly populations

Preferred Qualifications

• Geriatric Nurse Practitioner certification
• Additional experience in geriatric care
