About the Role
The Resource Nights & Weekend (ReNW) Nurse Practitioner (NP) is an active member of Chapters Health System’s Medical Staff and provides fully remote virtual serious illness care management to patients and families throughout our continuum of care. This continuum may include but is not limited to home-based, facility-based, and inpatient adult and pediatric hospice; palliative care; PACE, and additional service lines and programs offered or supported by Chapters Health System.
Responsibilities
- Provides fully remote virtual serious illness care management to patients and families throughout our continuum of care, including home-based, facility-based, and inpatient adult and pediatric hospice; palliative care; PACE, and other service lines.
- Consults with medical staff and/or other members of the interdisciplinary team (IDT) to ensure optimal patient care in an effective and efficient manner.
- Performs virtual assessment of patient/family/caregiver needs, identifies, manages, orders, and prescribes medication and equipment as appropriate.
- Adjusts and supports the plan of care in accordance with Chapters policies and procedures and within the guidelines of the corresponding nurse practice act.
- Assesses the caregiver’s ability to meet the patient’s immediate needs and confidence in the patient’s plan of care; provides education to increase caregiver confidence as necessary.
- Supports staff by answering clinical questions, entering orders and prescriptions, and addressing the clinical needs of patients and families.
- Maintains access to all electronic medical records (EMR), electronic prescribing systems, and other required websites and applications; documents all interactions in the EMR in a timely manner.
- Communicates and collaborates with the primary team to ensure continuity of care.
- Supports discharge planning, including prescribing medications, entering discharge orders and instructions, etc.
- Participates in Quality Assurance/Performance Improvement (QAPI) activities, including regular medical record reviews and medical peer reviews.
- Seeks to continually improve care practices to assure compliance with regulatory and accreditation standards.
- Maintains current knowledge of the latest research and trends in Hospice and Palliative Medicine; obtains continuing education as required for licensure.
- Obtains and maintains hospital/facility credentialing as required; participates in departmental and medical staff activities as requested.
- Communicates and collaborates with team members, patients/families/caregivers, and partnering providers within HIPAA guidelines.
- Provides patient and family education to maintain quality care, decrease anxiety, and facilitate living with the highest quality of life.
- Participates as a member of the interdisciplinary team to develop and implement a patient-directed individualized plan of care.
- Functions as a role model in providing exceptional customer service through effective and timely communication.
- Demonstrates flexibility in providing coverage and availability; supports organizational changes.
- Achieves expected productivity standards as defined by the organization and in alignment with best practices.
- Implements time-based billing procedures using the EMR system and other methods to capture timely billing (weekly for home and nursing home; daily for inpatient and clinic charges).
Requirements
- Minimum of 3 years of recent clinical experience, including substantial experience in hospice, palliative care, and/or acute inpatient care.
- Master’s or Doctoral degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
- National certification, licensure, and prescriptive authority from the appropriate governing body for advanced nursing practice in the home state and additional state(s) where care is provided.
- Licensure and prescriptive authority must be obtained and maintained for all required jurisdictions; applications for required licenses must be submitted within 60 days of hire.
- Basic Life Support certificate from an approved organization and compact RN license in the home state by date of hire.

- Ability to apply common sense understanding to carry out instructions and advanced clinical judgment to problem-solve.
Physical Demands
- Regularly required to sit, speak, and hear.
- Required to use hands to finger, handle, or feel; reach with hands and arms.
- Specific vision abilities include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Work Environment
- Office environment requiring maintenance of a HIPAA-compliant, temperature-controlled, noise-free workspace.
- Must maintain reliable HIPAA-compliant high-speed (100mb) internet with hardwired backup, along with access to copiers, fax, scanner, shredder, etc.
- Patient privacy must be protected at all times, including using headsets for verbal communication and company-approved devices, websites, and applications.
Schedule
Shifts may vary and can include nights, evenings, weekends, and/or holidays based on the needs of the business.
Pay
Pay Rate: $52.50 per hour.
This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as ongoing compliance with the Drug-Free Workplace Policy. All Chapters Health System employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made, as well as ongoing eligibility.
