Registered Nurse - Per Diem Weekends

About the role

Provide personalized nursing care in patients' homes through home health visits, supporting clients through recovery from injury, illness, or surgery to help them meet their health goals.

Responsibilities

• Assess patients, develop care plans, and deliver skilled services including medication management and wound care.
• Educate patients and families to promote recovery and health management.
• Travel locally to various cases as assigned.
• Work closely with an interdisciplinary care team, providing 1:1 direct care.

Requirements

• Active and current RN license.
• Preferred: one year of clinical experience; home health experience is a plus but not required (training provided).
• Ability to travel locally.
