Nurse Practitioner/ Collaborative with Medical Director SNF

Responsibilities

• Act as a Skilled Nursing Facility’s leading provider of internal medicine services
• Collaborate with the facility’s interdisciplinary team and physicians
• Serve as a day-to-day clinical leader
• Help oversee the care of all of the facility’s residents
• Work with the entire facility’s patient population - short and long-term residents
• Oversee their day-to-day care with the assistance of the facility’s nursing staff
• Guide the care of residents with the support of the facility’s medical directors and attending physicians
• Provide types of care, including but not limited to:
• Comfortable and proficient with documentation and CPT coding
• Evaluation of new-admission visits
• Coordination of discharge visits
• Monthly stability visits for long term residents
• Evaluations for patients exhibiting changes in condition
• Advanced Care Planning

Requirements

• Licensure / Certification / Registration: Advanced Practice Licensure (APN) with an applicable state license
• Must have an active license and DEA #
• Previous experience in nursing or other medical fields (preferred)
• Minimum one-year experience in a Skilled Nursing setting
• Ability to build rapport with patients and their loved ones
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Strong leadership experience with emphasis on teamwork in a healthcare setting
• Master’s degree required
