Nurse Practitioner-Bilingual English/Creole

Responsibilities

• Records physical findings and formulates plan and prognosis based on patient's condition.
• Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribes or recommends drugs or other forms of treatment.
• Monitors and maintains progress of patients and reports progress to supervising physician.
• Works collaboratively with hospice employees and volunteers as part of an interdisciplinary team (IDT).
• Assists in lifting and transferring patients as required, including lifting objects in excess of 20 pounds.
• Performs physical tasks such as standing or walking in excess of one hour per day, talking, hearing, sitting, stooping, kneeling, and crouching as needed.

Requirements

• Minimum of 2 years experience in hospice, home health, or community health within the past 5 years.
• Specific vision abilities including close vision, distance vision, and ability to adjust focus.

Qualifications

• Completion of an accredited Nurse Practitioner program.
• State licensure as a Nurse Practitioner.
