Compensation and Benefits
- Up to $155,000 annual salary based on experience

Responsibilities and Facility Details
- 100% outpatient role with primary focus on geriatrics patient population
- Preventative care emphasis on chronic conditions to reduce hospitalizations
- Part of a well-trusted, physician-led healthcare organization with over 120+ locations in 15 states
- Patient-focused with a small patient panel of up to 450 patients
- Monday–Friday schedule, no weekend responsibilities
- Dedicated care team for full support
- On-site lab, pharmacy, and X-ray services available
Qualifications and Skills
- Board Certified or Board Eligible as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
- 2+ years of practice experience preferred
- Bilingual providers fluent in Spanish encouraged to apply
