Responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with your care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care with support from quality-based coders.
- Participate in on-call rotation and support growth plans at your center.
Requirements
- Master's Degree in Nursing or completion of a PA program with board certification.
- An active, unrestricted NP or PA license in the state of practice.
- Screened for TB if selected for this role.

- Experience providing value-based care.

Pay
$123,800 - $170,300 per year
